MTA IDENTIFICATION CARDS

To utilize the Mililani Recreation Center facilities, the individual must obtain a valid MTA membership I.D. card from the Recreation Center III Office.

  1. Membership I.D. cards will be issued only to homeowners, persons residing at the homeowner's address, renters, and houseguests residing at the MTA property. Proof of residency at the MTA property may be required. A temporary membership card will be issued if the individual requesting a card is not listed in the MTA files; a regular membership card will be issued when proof of ownership is provided.

  2. Photo I.D. pictures can be taken at the Mililani Recreation Center III's office during the "Office Information Hours" as stated in the MILILANI TOWN RULES.

  3. Appropriate membership request forms completed by the homeowner and applicable fee payments must be submitted before membership I.D. card photos will be taken.

  4. A driver's license or State I.D. is required to identify the member for photo I.D.-taking. In the case of minors (17 yrs. and younger) without a photo I.D., a parent or guardian must accompany the minor for the purpose of I.D. certification.
  5. Completed membership card must be picked-up by the respective member or a member of the household.
  6. The issued membership I.D. card is the property of Mililani Town Association. It is not transferable and can be recalled or canceled at any time without prior notice.
  7. Membership I.D. cards become void if lost, stolen, upon expiration, sale of the MTA property, transfer of privileges, or if replaced with another card.
  8. Privileges for the use of the Mililani Recreation Center with the membership I.D. card is subject to the MILILANI TOWN RULES.
  9. Misrepresentation of facts to obtain a membership I.D. card or the use of said I.D. card to obtain Recreation Center privileges contrary to the rules as stated in the MILILANI TOWN RULES may result in the rescinding of membership I.D. cards and/or suspension of privileges for the member(s) of the respective MTA property.

"MEMBER" CARD POLICY

  1. The homeowner must submit a signed "Member Card Request" form listing the individuals for which cards are being requested and stating that said individuals are residing at the homeowner's property address.
  2. "Homeowner" cards shall be issued at a nominal fee to homeowners of a MTA property. These cards shall be issued only to individuals residing at the MTA property address.
  3. Homeowner(s) must return all "Homeowner" cards upon expiration, sale of the property, or transfer of privileges to a renter.
  4. Homeowners with "Homeowner" cards will be entitled to all Mililani Recreation Center privileges with the exception of the Youth Center facility which will require the payment of an additional fee.

"RENTER" CARD POLICY

  1. The homeowner must submit a signed "Member Card Request" form listing the individuals for which cards are being requested and stating that said individuals are residing at the homeowner's property address.
  2. "Homeowner" cards shall be issued at a nominal fee to homeowners of a MTA property. These cards shall be issued only to individuals residing at the MTA property address.
  3. Homeowner(s) must return all "Homeowner" cards upon expiration, sale of the property, or transfer of privileges to a renter.
  4. Homeowners with "Homeowner" cards will be entitled to all Mililani Recreation Center privileges with the exception of the Youth Center facility which will require the payment of an additional fee.

"RENTER" CARD POLICY

  1. The homeowner or agent of a MTA property must submit a completed "Renter Card Request" form with a copy of the lease agreement either in person or with a notarized letter. Said agreement must indicate the names of the renters residing at the property address. The property homeowner or agent must return all "Homeowner" or previously issued "Renter" membership cards issued for the property being leased. By doing so, the homeowner relinquishes all membership privileges to the Mililani Recreation Center and transfers said privileges to the designated renter(s). "Renter" cards will not be issued until all outstanding "Homeowner" cards and previously issued "Renter" cards have been returned to MTA.
  2. "Renter" cards shall be issued at a nominal fee to the renters as designated by the homeowner of the MTA property or its agent.
  3. Each "Renter" card shall be valid for one (1) year or the term of the rental lease agreement, whichever is shorter. All "Renter" cards must be returned immediately upon expiration or termination of the rental lease agreement, whichever takes place first.
  4. Renters with "Renter" cards will be entitled to all Mililani Recreation Center privileges with the exception of the Youth Center facility which will require the payment of an additional fee.

"HOUSEGUEST" CARD POLICY

  1. Any homeowner, including the landlord of any renter card-holder, may request in writing issuance of "Houseguest" membership card privileges for not more than five (5) invites who are actually residing temporarily at the MTA property address of the homeowner.
  2. The homeowner will be assessed a nominal fee per "Guest" membership card issued.
  3. "Guest" card privileges will be valid for not more than three (3) months or the duration of the continuous residence, whichever is shorter.
  4. Guest(s) do not have privileges to invite guests into the Mililani Recreation Center.
  5. Guests with "Guest" cards will be entitled to all Mililani Recreation Center privileges with the exception of the Youth Center facility which will require the payment of an additional fee.

"GUEST" POLICY

  1. Guests without a "Guest" card must be accompanied by a MTA member to be admitted and to remain in the Mililani Recreation Center facility. Any member who fails to remain on the premises with an invited guest shall forfeit all privilege to use the facility for the day and the guest(s) will be ejected from the premises.
  2. Non-adult members (17 yrs. and younger) will not be granted privileges to bring in guests to use the MTA swimming pools.
  3. Members are limited to four (4) guests per adult (homeowner/renter) cardholder at any given time.
  4. Guests are not permitted to participate in any program/classes or MTA activities (unless so wavered by the Recreation Director), nor reserve the tennis courts for playing time, rent facilities, or check out MTA equipment.
